ABSTRACT

Introduction: People with multimorbidity and their caregivers are beginning to be recognized as emerging subjects of health systems. In Colombia there is no differentiated approach to care for this population, as well as its health-disease process. Objective: To understand the experience of people with multimorbidity and their caregivers after receiving a case management intervention. Methods and materials: It is a qualitative study in which 33 participants among people with multimorbidity and their caregivers who received intervention with case managers were interviewed, a comparative analysis and use to tools analytics grounded theory. Results: There are 3 dimensions that are, the actors where nursing becomes relevant as a reliable source of care; the Care Meeting, as a space created within case management to maintain trust and; Results in the health system, where the need to integrate this type of outbreak into the Colombian Health Model is confirmed. Discussion: Complementary qualitative evidence data from the central study with a greater impact on the quality of care through the therapeutic relationship at home. Conclusion: The dyad requires home support for self-management of the disease based on trust, empathy, empowerment and administrative management carried out by case managers.


Introducción: Las personas con multimorbilidad y sus cuidadores empiezan a ser reconocidos como sujetos emergentes en los sistemas de salud. En Colombia no existe un abordaje diferenciado para la atención de esta población, así como de su proceso de salud-enfermedad. Objetivo: Entender las experiencias de personas con multimorbilidad y sus cuidadores tras recibir una intervención de gestión de casos. Materiales y métodos: Estudio cualitativo en el que se entrevistó a 33 participantes con multimorbilidad y sus cuidadores que recibieron una intervención por parte de gestores de casos; se realizó un análisis comparativo y se utilizaron herramientas analíticas de la teoría fundamentada. Resultados: Existen tres dimensiones: los actores, donde la enfermería cobra relevancia como fuente confiable de cuidado; la reunión de atención o cuidado, como un espacio creado en la gestión de casos para mantener la confianza; y resultados en el sistema de salud, donde se confirma la necesidad de integrar este tipo de avance en el modelo de salud colombiano. Discusión: Los datos cualitativos complementarios del estudio central evidencian un mayor impacto en la calidad del cuidado a través de la relación terapéutica en el hogar. Conclusión: La díada requiere acompañamiento domiciliario para la autogestión de la enfermedad que esté basado en la confianza, la empatía, el empoderamiento y la gestión administrativa llevada a cabo por los gestores de casos.


Introdução: As pessoas com multimorbidade e seus cuidadores estão começando a ser reconhecidos como sujeitos emergentes dos sistemas de saúde. Na Colômbia, não há uma abordagem diferenciada para o atendimento dessa população, bem como para seu processo saúde-doença. Objetivo: Compreender a experiência de pessoas com multimorbidade e seus cuidadores após receberem uma intervenção de gerenciamento de casos. Métodos e materiais: Trata-se de um estudo qualitativo no qual foram entrevistados 33 participantes entre pessoas com multimorbidade e seus cuidadores que receberam intervenção com gerentes de caso, uma análise comparativa e o uso de ferramentas analíticas da teoria fundamentada. Resultados: Existem três dimensões que são: os atores onde a enfermagem se torna relevante como uma fonte confiável de cuidados; a Reunião de Cuidados, como um espaço criado dentro do gerenciamento de casos para manter a confiança e; Resultados no sistema de saúde, onde a necessidade de integrar esse tipo de surto no modelo de saúde colombiano é confirmada. Discussão: Dados complementares de evidências qualitativas do estudo central com maior impacto na qualidade do atendimento por meio da relação terapêutica no domicílio. Conclusão: A díade requer apoio domiciliar para o autogerenciamento da doença com base na confiança, empatia, empoderamento e gerenciamento administrativo realizado pelos gerentes de caso.

ABSTRACT

Objective: Choanal atresia (CA) is a rare congenital malformation caused by the obliteration of the posterior choanae by an atretic plate. The aim of our study is to describe the diagnosis and management modalities of CA and to determine the factors associated with recurrence. Materials and methods: This is a retrospective study based on the medical records of patients with CA managed in our department in the period between 2002 and 2021. We studied the clinical features and management modalities of each patient. For patients who developed a recurrence, we determined the factors associated with recurrence based on a bivariate analysis. Results: We studied the medical records of 26 patients with either a bilateral (n=8) or a unilateral (n=16) form of CA. The median age at surgery was two days for bilateral forms and 5 years and 4 months for unilateral forms. At computed tomography scan, CA was mixed (n=20), bony (n=4) or membranous (n=2). All patients underwent intranasal endoscopic surgical treatment using cold instruments alone in membranous forms and combined to the drilling of the atretic plate in bony and mixed forms. The surgical management included the resection of the posterior part of the vomer bone and the placement of nasal stents in 10 and 16 patients respectively. We recorded 6 cases of recurrence requiring a surgical re-intervention. The presence of associated cranio-facial malformations was the only factor associated with recurrence (p=0,001). Conclusion: Choanal atresia diagnosis was based on nasal endoscopy and CT scan. Surgical treatment using transnasal endoscopic approach was an effective and safe technique. Associated local malformations was a factor associated with re-stenosis

ABSTRACT

Background: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) caused coronavirus disease 2019 (COVID-19) pandemic with major disruptions globally. Northwest Province Department of Health (NWDoH) in South Africa set up comprehensive epidemiological emergency response plans for preventing, finding, containing and stopping the spread of COVID-19 in accordance with the National Disaster Management Act. Objectives: This After-Action Report (AAR) describes the provincial response to the pandemic from September 2020 to October 2022. Method: The AAR was conducted using the World Health Organization AAR methodology. Focus groups discussed five items: coordination, leadership and governance; epidemiology, surveillance and laboratory; case management and continuity of essential services; risk communication and community engagement and COVID-19 vaccination. Results: The timely establishment and activation of provincial intergovernmental and intersectoral coordinating structures led to effective coordination, resource mobilisation, leadership, decision-making and intervention. The effective communication in the department and other stakeholders resulted in improved surveillance data quality, timelier response and increased ownership of data. Dissemination, training and implementation of case management protocols ensured standardised case management. The multi-channel information dissemination targeting different audiences empowered people with real-time knowledge on the infection and encouraged health-seeking behaviours. Conclusion: The AAR demonstrated the importance of coordinated epidemiological, laboratory and communication response that requires significant public health reserve capacity in peacetime for rapid expansion in an emergency. Contribution: This review contributes to the body of knowledge emerging from the COVID-19 pandemic and provides guidance on enhanced public health response to future emergencies.

Objetivo: relatar a experiência de elaboração de um algoritmo de gerenciamento de casos para pessoas com hipertensão arterial sistêmica atendidas na atenção primária. Método: relato de experiência em que a construção do algoritmo foi baseada no modelo da Community Access Ageing, Disability and Home Care, Department of Human Services NSW9, que prevê sete etapas: Compromisso, Avaliação, Planejamento, Implementação, Monitoramento, Revisão e Encerramento. Resultados: compuseram o algoritmo questionários validados e atividades como visitas domiciliares, consultas de enfermagem, plano terapêutico individualizado e pactuação de metas, educação em saúde, contato telefônico e redirecionamento para a rede atenção à saúde. O tempo de elaboração foi de 12 meses. Conclusão: o algoritmo desenvolvido representa uma ferramenta simples e dinâmica de gerenciamento de casos, que orienta as atividades de cuidado de pessoas com hipertensão atendidas na atenção primária, mediante sete etapas, e facilita a leitura dos resultados.


Objective: to report the experience of elaborating a case management algorithm for people with systemic arterial hypertension treated in primary care. Method: experience report in which the construction of the algorithm was based on the model of Community Access Ageing, Disability and Home Care, Department of Human Services NSW9, which provides for seven stages: Commitment, Evaluation, Planning, Implementation, Monitoring, Review and Closure. Results: validated questionnaires and activities such as home visits, nursing consultations, individualized therapeutic plan and goal agreement, health education, telephone contact and redirection to the health care network were included in the algorithm. The preparation time was 12 months. Conclusion: the algorithm developed represents a simple and dynamic case management tool that guides the care activities of people with hypertension treated in primary care, through seven stages, and facilitates the reading of results.


Objetivo: relatar la experiencia del desarrollo de un algoritmo de manejo de casos para personas con hipertensión arterial sistémica atendidas en atención primaria. Método: relato de experiencia en el que la construcción del algoritmo se basó en el modelo Community Access Aging, Disability and Home Care, Department of Human Services NSW9, que prevé siete pasos: Compromiso, Evaluación, Planificación, Implementación, Monitoreo, Revisión y Cierre. Resultados: cuestionarios validados y actividades como visitas domiciliarias, consultas de enfermería, plan terapéutico individualizado y acuerdo de metas, educación para la salud, contacto telefónico y redirección a la red de salud compusieron el algoritmo. El tiempo de preparación fue de 12 meses. Conclusión: el algoritmo desarrollado representa una herramienta de gestión de casos simple y dinámica, que orienta las actividades de atención de las personas con hipertensión arterial asistidas en la atención primaria, a través de siete pasos, y facilita la lectura de los resultados.

Objetivo: Avaliar a satisfação da assistência de enfermagem sob a ótica de pacientes com Crohn atendidos pelo programa de gerenciamento de caso.Métodos: Estudo quase experimental com grupo único, do tipo antes e depois, realizado com 36 pacientes comdoença de Crohn, no período de maio a setembro de 2017 no ambulatório de gastroenterologia do hospital Universitário do Piauí. Utilizou-se a SERVQUAL adaptada para avaliar a satisfação antes e após a intervenção de programa de gerenciamento de caso, que se caracteriza como programa de cuidado paraavaliar, gerenciar, ofertarplano de cuidado e monitoraros pacientes por um período de seis meses.Resultados: OGap (lacuna) total, que é a diferença entre a expectativa com o serviço prestadoe a sua percepção, assumiu valorpositivo (0,20±0,9). As percepções dos pacientes de Crohn superaram as expectativas, isto é, os pacientes se consideraramsatisfeitos com a qualidade do serviço de enfermagem após intervenção nas dimensões confiabilidade(0,3 ± 1,3), atendimento (0,2 ± 1,1)e empatia (0,5 ± 1,5), encontrando-se,porém insatisfeitos com a segurança (-0,1±1,0).Conclusão: Houve aumento da satisfação dos pacientes com doença de Crohn após intervençãono que se refere àconfiabilidade, atendimento eempatiaoferecidadurante o programa de gerenciamento de caso


Objective: To evaluate the satisfaction of nursing care from the perspective of patients with Crohn's disease attendedby the case management program.Methods: Aquasi-experimental study with a single group, before and after, was conducted with 36 Crohn's patients,in the period from May to September 2017 in the gastroenterology outpatient clinic of the University Hospital of Piauí. The adapted SERVQUAL was used to assess satisfaction before and after the Case Management program intervention, which is characterized as a care program to assess, manage, offer care plans, and monitor patients over a six-month period.Results: The total Gap, which is the difference between the expectation ofthe service provided and its perception, assumed a positive value (0.20.2±0.9). The perceptions of Crohn's patients exceeded expectations, that is, they were satisfied with the quality of nursing service after intervention in the dimensions:reliability (0.3 ± 1.3), attendance(0.2 ± 1.1),and empathy (0.5 ± 1.5);but dissatisfied with safety (-0.1 ± 1.0).Conclusion: There was an increase in Crohn's disease patients' satisfaction after intervention regarding the reliability, attendance, and empathy offered during the case management program

ABSTRACT

La gestion du malade dans l?univers hospitalier implique non seulement le personnel soignant, mais aussi d?autres acteurs sociaux encore moins connus des gestionnaires des structures hospitalières et des catégories socio professionnelles y évoluant. Parmi ces acteurs encore moins connu, il y a le garde-malade faisant partie du personnel hospitalier en charge de la prise en charge des patients en milieu hospitalier. Cependant au Centre Hospitalier et Universitaire de Brazzaville en République du Congo, les garde-malades sont constitués des parents, amis et connaissances dont la représentation est importante dans cet univers hospitalier. Cette représentation aussi remarquable soit ­ elle, interpelle plus d?un observateur au point de s?interroger sur les raisons à l?origine de cette présence. Notre étude répond à la logique qui est celle de tenter de comprendre et d?analyser ce phénomène devenu très inquiétant. Ce phénomène est donc à lire dans la perspective de la complexité des réalités sociales qui s?expliquent à partir des déterminants socio culturels, fondements de la vie sociale. Aussi, la faiblesse des services sous tutelle du C.H.U-B et la culture bantoue, laquelle repose sur la solidarité mécanique qui, appelle à la cohésion sociale, constituent des facteurs qui expliquent ce phénomène. A cet effet, une observation participante a été effectuée dans cette structure hospitalière. Celle-ci a permis de relever une influence importante qui, pour diverses raisons réponde aux préoccupations des malades. Cette étude présente trois intérêts : scientifique, social et personnel. D?abord, elle montre la difficulté en ressources humaines qui se traduit par la sous représentativité des catégories socio professionnelles qui, contribueraient à résoudre la question de l?effectivité de prise en charge du malade. Ensuite, l?étude présente un autre intérêt qui met en exergue la cohésion sociale qui, s?exprime à travers la chaine de solidarité dont la famille est le maillon. Le dernier centre d?intérêt est personnel. Il met en relief un aspect culturel très intéressant, celui de l?homogénéité et de la dynamique des rapports des acteurs impliqués dans la gestion du malade. Quoi qu?il en soit, le garde-malade joue un rôle de premier plan dans la gestion du malade. Il accompagne le personnel soignant, car partageant le même environnement social que le malade. Aussi, sa visibilité nécessite un encadrement juridique pour lui donner une véritable identité.


The management of the patient in the hospital word involves not only the nursing staff, but also other social actors even less known to the manager of hospital structures and socio-professional categories. Among these actors even less known, there is the nurse who is part of the hospital staff in charge of the care of patients in hospital environment. Howerver at the Brazzaville Hospital and University Center in the Republic of Congo, the nurses are made up of relatives, friends and acquaintanees whose representation is important in this hospital environment. This representation, as remarkable as it is, calls out to more than one observer to the point of questioning the reasons behind this presence.Our study reponds to the logic of trying to understand and analyze this phenomenon, which has become very worrying. This phenomenon should therefore be read in the perspective of the complexity of social realities which can be explained on the basis of socio-cultural determinants, the foundations of social life. Also, the weakness of the services under the supervision of the C.H.U-B and of the Bantu culture, which is based on mechanical solidarity, which, calls for social cohesion, are factors that explain this phenomenon. To this end, a participant observation was carried out this hospital structure. This made it possible to identify an important influence which, for various reasons, responds to the concerns of patients. This study has three interests: scientific, social and personal. First, it shows the difficulty in human resources which result in the representativeness of the socio-professional categories which would help to resolve the issue of effectiveness of patient care. The study presents another interest which highlights the social cohesion which is expressed through the chain of solidarity of which the family is the link. The last area of interest is personal. It highlights a very interesting cultural aspect, that of the homogeneity and dynamics of relationships of the actors involved in the management of the patient. In any case, the nurse plays a leading role in management of the patient. He supports the nursing staff, because they share the same social environment as the patient. Also, its visibility requires a legal framework to give it a real identity

BACKGROUND AND OBJECTIVES: Coronavirus disease 2019 (COVID-19) is a global pandemic. Older people and those with poorly controlled co-morbidities have higher risk of mortality. This study was conducted to highlight the clinical features, challenges of management and outcome for the patients we have seen in our centre over the past one year. METHODS: This was a retrospective cross-sectional study involving all patients admitted in the COVID-19 Isolation unit of University of Uyo Teaching Hospital (UUTH) from June, 2020­May, 2021. Clinical and laboratory information were obtained from the patient case notes. Ethical clearance for the conduct of the study was obtained from the Ethics committee, UUTH, Uyo. Data was analysed with STATA version 13. RESULTS: Thirty-three (37.9%) patients were COVID-19 PCR positive. The mean ± SD age of COVID-19 PCR positive patients was 57.3 ± 13.4 years with majority (69.7%) being above 50 years. There was a male preponderance (75%). Eleven (34.4%) patients died while 21(65.6%) were discharged. The highest co-morbidity associated with COVID-19 mortality was diabetes mellitus (7 out of 11; 63.6%). There was a poor uptake of supportive investigations for the management of COVID-19 patients. A raised body temperature (P=0.0006), a low SPO2(0.00004), high respiratory rate (0.0009) on admission and shorter duration of admission (0.0002), were associated with mortality. CONCLUSION: The presence of co-morbidities, fever, low SPO2 and high respiratory rates on admission are associated with increased mortality from COVID-19 disease. A paucity of supportive investigations was a major challenge to COVID-19 management. We therefore recommend the strengthening of our laboratory capacity.

ABSTRACT

Se realizó una investigación de tipo cuantitativa, observacional, descriptiva y transversal en adultos ingresados en el Hospital General Alfredo Noboa Montenegro, de Guaranda, Ecuador, desde el 1ro. de julio al 30 de septiembre del 2020. Se realizó una caracterización clínica y de gravedad de adultos con COVID-19 hospitalizada. Se encontró que en el caso de presentar enfermedad cardiometabólica se aumenta la probabilidad de presentar los indicadores de gravedad (p<0,05). Presentar otras enfermedades asociadas (no cardiometabólicas) también incrementó la probabilidad de presentar indicadores de gravedad con un nivel de significación del 5%, al igual que padecer de asma bronquial (p<0,05 para ambas características); en el caso del tabaquismo solo fue estadísticamente significativo el aumento de la probabilidad de ingreso en el hospital, de presentar neumonía y de fallecer (p<0,05 para las tres variables). Los resultados de este estudio pueden usarse para identificar los factores de riesgo asociados con un mal pronóstico en pacientes con COVID-19 en Ecuador y ayudar a desarrollar medidas preventivas para disminuir la carga de la enfermedad y trazar estrategias para el manejo de los casos(AU)


A quantitative, observational, descriptive and cross-sectional investigation was carried out in adults admitted to the Alfredo Noboa Montenegro General Hospital, in Guaranda, Ecuador, from the 1st. from July to September 30, 2020. A clinical and severity characterization of hospitalized adults with COVID-19 was carried out. It was found that in the case of presenting cardiometabolic disease, the probability of presenting severity indicators is increased (p<0.05). Presenting other associated diseases (not cardiometabolic) also increased the probability of presenting severity indicators with a significance level of 5%, as well as suffering from bronchial asthma (p<0.05 for both characteristics); In the case of smoking, only the increase in the probability of hospital admission, pneumonia and death was statistically significant (p<0.05 for the three variables). The results of this study can be used to identify risk factors associated with a poor prognosis in patients with COVID-19 in Ecuador and help develop preventive measures to reduce the burden of the disease and devise strategies for case management(AU)

ABSTRACT

Desde época temprana de la cirugía cardíaca (CC), la fibrilación auricular (FA) ha sido un acompañante frecuente del posoperatorio, y no es esperable su abatimiento en el futuro cercano. La interpretación de su significado clínico se ha modificado en los últimos años, tras conocerse su tendencia recurrente y su asociación con serias complicaciones inmediatas y a largo plazo. Esto deja entrever un nuevo desafío, dejando de ser un problema menor y de consideración puntual en el perioperatorio para constituir un tema de preocupación y seguimiento en el futuro alejado, aún con incertidumbres evolutivas y de manejo. La profilaxis efectiva de esta arritmia, una respuesta lógica al problema, es dificultosa por la multiplicidad de factores de riesgo y lo intrincado de su génesis, todavía no completamente dilucidada, sumadas a la edad creciente de los pacientes intervenidos, la complejidad mayor de los procedimientos, los posibles efectos colaterales de los fármacos empleados y la inexistencia de un algoritmo predictivo confiable que permita racionalizar las medidas preventivas. Además, muchas recomendaciones de las guías de práctica clínica actuales se basan en información obtenida en estudios realizados en la FA primaria, por lo que su adopción en el escenario de la CC ha sido menor a la deseable. Todos estos aspectos son objeto de análisis en esta revisión que finaliza con pautas de manejo práctico de la arritmia en el entorno perioperatorio.


Since an early age of heart surgery, atrial fibrillation has been a frequent companion of the postoperative period, and its decline is not to be expected in the near future. The interpretation of its clinical significance has changed in recent years, after knowing its recurrent trend and its association with serious immediate and long-term complications. This fact unveils a new challenge, as it is no longer a minor problem of consideration restricted to the perioperative period and has become a topic of concern and follow-up in the distant future, still with uncertainties as to its evolution and management. The effective prophylaxis of this arrhythmia, a logical response to the problem, has been difficult by the multiplicity of risk factors and the intricate of its genesis, not yet completely elucidated, added to the increasing age of the patients involved, the greater complexity of the procedures, the possible side effects of the drugs used and the absence of a reliable predictive algorithm that could allow to rationalize preventive measures. In addition, many recommendations from current clinical practice guidelines are based on information obtained from studies in primary atrial fibrillation, so their adoption in the heart surgery scenario has been less than desirable. All these aspects are analyzed in this review, which ends with directives for the practical management of the arrhythmia in the perioperative environment.


Desde os primeiros días da cirurgia cardíaca, a fibrilação atrial (FA) tem sido uma companheira frequente para o pós-operatório, e sua reduçao não é esperada em um futuro próximo. A interpretação de sua significância clínica mudou nos últimos anos, tendo conhecido sua tendência recorrente e sua associação com sérias complicações imediatas e de longo prazo. Este fato mostra um novo desafio, pois deixou de ser um pequeno problema e uma consideração oportuna no perioperatório para constituir um tema de preocupação e acompanhamento em um futuro distante, mesmo com incertezas quanto à sua evolução e gestão. A profilaxia efetiva dessa arritmia, uma resposta lógica ao problema, tem sido cercada pela multiplicidade de fatores de risco e pela intrincação de sua gênese ainda não completamente elucidada, juntamente com a idade crescente dos pacientes envolvidos, a maior complexidade dos procedimentos, os possíveis efeitos colaterais dos medicamentos utilizados e a ausência de um algoritmo preditivo confiável para racionalizar as medidas preventivas. Além disso, muitas recomendações das guias atuais de prática clínica são baseadas em informações obtidas em estudos conduzidos em FA primária, de modo que sua adoção no cenário da cirurgia cardíaca tem sido menos do que desejável. Todos esses aspectos são analisados nesta revisão, que termina com diretrizes práticas de gestão para arritmia no ambiente perioperatório.

ABSTRACT

Objective. To synthesize the best available evidence regarding the effectiveness of nursing case management in primary health care, compared to usual care, in improving blood pressure in adults over 18 years with hypertension. Methods. systematic review that includes studies carried out with adult patients diagnosed with hypertension, with or without other concomitant chronic diseases, followed-up by a case manager nurse, who evaluated the effectiveness of case management in the improvement of blood pressure. A critical evaluation of the studies was made and the results of interest were described using the instruments and tools from the Joanna Briggs Institute. Due to the heterogeneity of the included studies, the results of similar measures were not grouped in statistical meta-analysis. A narrative and tabular format was used to synthesize and present them. Results. Six randomized controlled trials were critically evaluated and included in the review. The total sample was 1963 participants. The results showed the outcomes compared at baseline and at the end of follow-up (six or twelve months). Regarding the main outcome, systolic and diastolic blood pressure, there was some reduction in the group followed-up through case management in studies lasting six months; however, the impossibility of comparing the findings poses limitations to answering the questions in this review. Conclusions. Despite the heterogeneity of the studies, the narrative and tabular analysis demonstrated that short-term case management in primary care (six-month studies) helped to reduce blood pressure levels, although the level of evidence for these results is low or very low.


Objetivo. Sintetizar las mejores evidencias disponibles sobre la efectividad del manejo de casos de enfermería en la atención primaria en salud en comparación con los cuidados habituales para mejorar la presión arterial en adultos mayores de 18 años con hipertensión arterial sistémica. Métodos. Revisión sistemática de estudios realizados en pacientes adultos con diagnóstico de hipertensión arterial, monitoreados por el enfermero gestor de casos quien evaluó la efectividad del manejo de la mejoría de la presión arterial con o sin otras enfermedades crónicas concomitantes. Se realizó una evaluación crítica de los estudios y se describieron los resultados de interés utilizando los instrumentos y herramientas del Instituto Joanna Briggs. Debido a la heterogeneidad de los estudios, los resultados de medidas similares no se agruparon en el meta-análisis estadístico. Se utilizó un formato narrativo y tabular para sintetizarlos y presentarlos. Resultados. Seis ensayos clínicos aleatorios fueron evaluados críticamente e incluidos en la revisión. La muestra total fue de 1963 participantes. Los resultados mostraron los hallazgos comparados al inicio y al final del seguimiento (seis o doce meses). En cuanto al resultado principal, la presión arterial sistólica y diastólica tuvo una reducción en el grupo acompañado por el gestor de manejo de casos en los estudios que duraron seis meses; sin embargo, la imposibilidad de comparar los hallazgos plantea limitaciones para responder las preguntas de esta revisión. Conclusión. A pesar de la heterogeneidad de los estudios, el análisis narrativo y de las tablas demostró que la gestión de casos por enfermería, a corto plazo (estudios de seis meses), en atención primaria, ayudó a reducir los niveles de presión arterial, aunque el nivel de evidencia para estos resultados fue bajo o muy bajo.

Foi realizada uma avaliação crítica dos estudos e descritos os resultados de interesse com a utilização dos instrumentos e ferramentas do Instituto Joanna Briggs. Devido à heterogeneidade dos estudos incluídos, os resultados de medidas semelhantes não foram agrupados na meta-análise estatística. Um formato narrativo e tabular foi usado para sintetizá-los e apresentá-los. Resultados. Seis ensaios clínicos randomizados foram avaliados criticamente e incluídos na revisão. A amostra total foi de 1963 participantes. Os resultados mostraram os desfechos comparados no início e no final do acompanhamento (seis ou doze meses). Em relação ao desfecho principal, pressão arterial sistólica e diastólica, houve alguma redução no grupo acompanhado pelo gerenciamento de casos em estudos com duração de seis meses; no entanto, a impossibilidade de comparar os achados impõe limitações para responder às perguntas desta revisão. Conclusões. Apesar da heterogeneidade dos estudos, a análise narrativa e tabular demonstrou que o gerenciamento de casos de curto prazo na atenção primária (estudos de seis meses) ajudou a reduzir os níveis de pressão arterial, embora o nível de evidência para esses resultados seja baixo ou muito baixo.

Método: Se trata de una investigación cualitativa descriptiva, llevada a cabo entre 13 participantes que sufrieron un Accidente Cerebrovascular, cuyo seguimiento fue gestionado por un enfermero durante seis meses y que fueron entrevistados en sus domicilios un mes después de haberse terminado el seguimiento. Las entrevistas se grabaron y se transcribieron en su totalidad y se analizaron con la ayuda de programas informáticos Resultados: De los 13 participantes, el 57,14% era del sexo masculino y el 50% tenía una edad comprendida entre los 50 y los 59 años. El análisis del contenido dio como resultado tres categorías: "Autogestión del cuidado", que se refiere a los cambios de actitud hacia el tratamiento, principalmente por el miedo a una recidiva en un período corto; la segunda trata de las "Secuelas y repercusiones tardías del Accidente Cerebrovascular", que presenta las dificultades de los participantes después del hecho, y la tercera se refiere al "Apoyo recibido después del Ictus", que hace hincapié en las acciones y actitudes del enfermero gestor del caso durante el período de seguimiento. Conclusión: La gestión del caso fue percibida por el participante como una herramienta de promoción de la salud, capaz de aumentar la adhesión al tratamiento.

Este resumen fue realizado a través de una búsqueda amplia de evidencia en MedLine® y EMBASE®, utilizando Ovid® como biblioteca virtual, a través de una estrategia de búsqueda, la cual fue previamente discutida y probada. La búsqueda se realizó el 11-02-2020, no se utilizaron filtros de idioma ni tipo de publicación. El criterio de inclusión fue cualquier tipo de publicación relacionada al nuevo Coronavirus 2019. La selección de evidencia y la extracción de datos se realizó por un revisor. La extracción de datos se realizó según una planilla previamente discutida que resumía las preguntas de interés, la cual fue necesario refinar en el proceso de revisión. Los resultados se presentan de manera descriptiva sin evaluar la certeza de la evidencia o calidad de la información.

ABSTRACT

La mordida abierta anterior es una maloclusión en que uno o más dientes no alcanzan el plano oclusal y no se establece contacto con sus antagonistas. Su tratamiento es difícil y existe controversia en la necesidad de un abordaje quirúrgico. El presente caso relata el manejo de un paciente femenino de 61 años de edad, atendido en la clínica del Instituto Mexicano de Ortodoncia, que acudió por "separación de sus dientes". La evaluación clínica y radiográfica presentó una clase I esquelética, biprotrusiva, con overbite de -30% con sonrisa baja y no consonante, deglución atípica como factor etiológico que permitió el diagnóstico de mordida abierta ante-rior. El tratamiento consistió en la colocación de brackets prescripción Roth slot .022 con tubos hasta terceros molares superiores e inferiores, colocando brackets más hacia gingival con respecto al centro de la corona anatómica de ambas arcadas para lograr la extrusión y cierre de la mordida abierta y arco de sonrisa; en cen-trales a 1.5mm, laterales a 1mm y caninos a 0.5mm. Fue fundamental el uso de topes oclusales posteriores y también de elásticos anteriores cortos y ligeros desde el primer día del tratamiento. Se consiguió la corrección de la posición de los dientes en un tiempo de 18 meses, que además se indujo un cambio en el patrón de deglución. Mediante el plan de tratamiento propuesto se demostró que se puede dar estabilidad a la oclusión y que no todo paciente con mordida abierta es candidato a tratamiento quirúrgico para la corrección de esta maloclusión.


The previous open bite is a malocclusion in which one or more teeth do not get the occlusal plane and no con-tact is made with their antagonists. Its treatment is difficult and there is controversy in the need for a surgical approach. The present case relates to the management of a 61-year-old female patient, treated at the clinic of the Mexican Institute of Orthodontics, who came for "separation of her teeth." The clinical and radiographic evaluation presented a skeletal class I, biprotrusive, with overbite of -30% with low and non-consonant smile, atypical swallowing as an etiological factor that allowed the diagnosis of anterior open bite. The treatment con-sisted of the placement of Roth slot .022 prescription brackets with tubes up to upper and lower third molars, placing more gingival braces with respect to the center of the anatomical crown of both arches to achieve extrusion and closure of the open bite and arch of smile; in centrals at 1.5mm, laterals at 1mm and canines at 0.5mm. The use of posterior occlusal bumpers and also of short and light anterior elastics from the first day of treatment was essential. Correction of the position of the teeth was achieved in a period of 18 months, which also induced a change in the swallowing pattern. Through the proposed treatment plan it was shown that oc-clusion stability can be given and that not every patient with an open bite is a candidate for surgical treatment for the correction of this malocclusion.


A mordida aberta anterior é uma má oclusão na qual um ou mais dentes não atingem o plano oclusal e não é feito contato com seus antagonistas. Seu tratamento é difícil e há controvérsia na necessidade de uma abord-agem cirúrgica. O presente caso refere-se ao tratamento de uma paciente de 61 anos de idade, atendida na clínica do Instituto Mexicano de Ortodontia, que veio pela "separação dos seus dentes". A avaliação clínica e radiográfica apresentou classe esquelética biprotrusiva I, com sobremordida de -30% com sorriso baixo e não consoante, deglutição atípica como fator etiológico que permitiu o diagnóstico de mordida aberta anterior. O tratamento consistiu na colocação de suportes de prescrição Roth .022 com tubos até os terceiros molares superiores e inferiores, colocando os suportes mais voltados para a gengiva em relação ao centro da coroa anatômica de ambos os arcos para obter extrusão e fechamento da mordida e arco aberto de sorriso; nas centrais a 1,5 mm, laterais a 1 mm e caninos a 0,5 mm. O uso de topes oclusais posteriores e também de elásticos anteriores curtos e leves desde o primeiro dia de tratamento foi essencial. A correção da posição dos dentes foi realizada em um período de 18 meses, o que também induziu uma alteração no padrão da deglutição. Através do plano de tratamento proposto, demonstrou-se que a estabilidade da oclusão pode ser dada e que nem todo paciente com mordida aberta é candidato ao tratamento cirúrgico para correção dessa má oclusão.

Abstract INTRODUCTION Malaria case management is a pivotal intervention in malaria elimination. However, many remote areas in Brazil still lack access to basic health services. This study describes a community-based approach (CBA) for malaria case management in the large remote area of the Jaú National Park (JNP), Amazonas, Brazil. METHODS In 2001, a general health CBA was initiated with a motor group (MG); a participative community health diagnosis (PCHD) was subsequently implemented between 2001 and 2005. In 2006, a CBA for malaria case management started with an expanded MG including all sectors with a stake in malaria control, from the local residents to the federal government. In 2008, community microscopists were selected and trained to diagnose hemoparasites. A full malaria strategy was implemented in 2009 with subsequent quality control follow-up. RESULTS Two educational materials were co-created with local communities. The MG identified malaria as a major health problem and the malaria MG planned the control activities. Ten communities selected a resident to become malaria microscopists, and ten solar-operated health centers were built. The number of slide readings increased from 923 in 2006 to 1,900 in 2009, while malaria infections decreased from 354 cases in 2005 to 20 cases in 2015. The excess time (≥ 48 hours) between first symptoms and diagnosis/treatment decreased from 68.9% of cases in 2005 to 14.3% in 2010. CONCLUSIONS While many factors were likely involved in the reduction of malaria transmission in the JNP, the CBA played an important role in the sustained success of the initiative.
